Voluntary Termination Pay Offer (VTP)

October 18, 2016 – November 11, 2016

Effective October 18, 2016, a Voluntary Termination Pay (VTP) offer is being made to

employees in the title of Customer Care Representative in GCA 463 in accordance with Article

25 of the CWA Operations collective bargaining agreement.

Eligible employees accepting this offer must be off payroll by November 11, 2016 (last day on

payroll).

Employees must complete the attached election form and submit their request of acceptance

or declination for the VTP offer to rhoadesm@avaya.com or fax to 720-444-1210.

You will receive an email confirmation once you have submitted your acceptance or declination

for this VTP offer.

The election period for this offer will be October 18, 2016 through November 1, 2016.

Employees must submit their request by end of day (11:59pm local time) on November

1, 2016, to be considered for this offer. Employees will be notified on November 2, 2016,

with the Company’s acceptance or declination of their Voluntary Termination Pay (VTP)

election.

The offer is limited within the designated GCA. If there are more requests than the limit in the

GCA location, an over-subscription may be considered. Employees eligible for this offer will be

selected in order of seniority by Geographic Commuting Area (GCA), pursuant to CWA Article

24.
